DATA PROTECTION

All information received will be kept by WadoKai Worcester, and used for the purpose of facilitating WadoKai Worcester training, external training courses and competitions. Information will be passed on to the National Governing Body (English Karate federation- EKF) for licencing purposes only, this means that at no point will you be contacted by the EKF directly. If the student no longer trains with WadoKai Worcester for longer than a year the information will be destroyed.

FIRST AID

In the event of an accident or injury, your permission is required for a qualified appointed first aid member of WadoKai Worcester or facility faculty to administer basic first aid. If the first aid required is more than basic, then an ambulance will be called and NHS personnel with deal with the situation at the scene, or if required by removing to hospital. If you do not sign, first aid will not be administered by any personnel and possibly may not be administered by the NHS.

LOSS OF POSSESSIONS

WadoKai Worcester Karate are not responsible for the loss of any possessions this includes clothing, licence books or any other equipment. If replacements of WadoKai Worcester equipment is required the cost of said item/s will be applied i.e licence replacement £30

POLICIES

I also agree to abide to WadoKai Worcester policies (which may be found on website: https://www.wadokaiworcester.co.uk/policies or provided upon request.) You are entitled to withdraw consent for first aid, photography and data holding and data sharing at any time.  Consent to abide by club policies is compulsory, membership to WadoKai Worcester will be withdrawn with immediate effect if this policy is not agreed to
